Duplicate Birth / Death Certificate
Requirements:
To obtain a copy of birth/death certificate, a search form must be completed and returned to the Kenya Embassy D.C.
Complete the Birth/Death Certificate Search Form and submit the completed form with the following:
- US $50 Money order or Cashiers Check Payable to Embassy of The Republic of Kenya.
- Copy of the original Birth/Death Certificate.
- A formal letter of Application.
- A self addressed stamped envelope for the return of your documents. The envelope should have enough postage for: FedEx, Express Mail, Certified Mail or Priority Mail with delivery confirmation (Note: Metered stamps and Regular mail are not acceptable).
